General Information Regarding Athletic Physicals
Per UIL and GISD rules and regulations, all student athletes, student managers (including statisticians, video filmers, class aids, etc.), student athletic trainers, transfer students, and foreign exchange students in grades 7-12 must have a completed Athletic Physical Form on file in the Athletic Trainer's office prior to any participation in practices, before, during, or after school (both in-season and out-of-season), or game/matches. Failure to do so will result in the student athlete not being allowed to participate in any GISD athletic sports. The Athletic Physical Form will be valid from April 1, 2024 through May 31, 2025.
The district offers free physical exams in the spring at the secondary campuses to help student athletes complete the physical. Also, parents and student athletes have the option of completing this requirement at the medical provider of their own choice – a physician, a physician assistant licensed by the State Board of Physician Assistant Examiners, a registered nurse as an advanced practice nurse by the Board of Nurse Examiners, or a doctor of chiropractic.
The Athletic Trainers will collect all completed forms, which will be kept on file. Parents may request copies if needed, allowing 48-72 hours for processing.
