Enrollment
Enrollment is on a first come, first serve basis. Completed enrollment forms and the enrollment fee need to be completed before a child's spot is reserved.
Please complete and submit:
- Admission Form
- Childcare Survey
- Non-Prescription Medication Form (as needed)
- Infant Feeding Instructions (for children 6 weeks to 12 months only)
- Photo Consent Form
- Parent Handbook (read and return the acknowledgement page, pg 36)
