Special Dietary Requests

Reasonable meal accommodations will be made if an allergy effects (1) a major life function, (2) is life threatening, or (3) a student’s disability restricts his/her diet.

The following form must be filled out in its entirety by a medical authority that’s licensed to write prescriptions by the state. The form must also have a parent/guardian signature before we can legally modify any student meals. 

Return completed form to your child's nurse or to the Child Nutrition department at the administration building.
